Travel Insurance is a good idea for a number of reasons. If you already have it, great! If not, we respect your decision. Either way, we’ll need you to fill out the waiver below in order for us to plan your trip.
Travel Insurance
I, the undersigned, am purchasing certain travel services through Miltonia Custom Travel, LLC (“Miltonia”) and am aware that certain risks are associated with such travel. Miltonia has provided me with information pertaining to certain travel insurance coverages available to me and those traveling with me, and that information on such insurance is available on the link provided therefore on Miltonia’s website www.MiltoniaTravel.com as well as from third-party insurance companies. I acknowledge that available travel insurance policies may include, without limitation, the following coverages: trip interruption and cancellation, emergency medical transportation, medical expense, lost or damaged baggage and personal belongings, and certain errors of travel vendors, travel suppliers, and/or other travel-related operators.
I hereby agree to and do release, and agree to indemnify and hold harmless Miltonia and its officers, members, managers, employees, agents, and representatives from and against any and all liabilities for personal injury, death, property damage, property loss, and/or other loss or harm to me and/or others traveling with me, including, without limitation, court costs and attorneys’ fees, directly or indirectly arising from and/or related to such travel. Miltonia has instructed me to contact a travel insurance company with any questions or concerns that I may have regarding travel protection.
